Who Qualifies for Chronic Care Management Services?
If you have Medicare and more than one chronic condition, you may be a candidate to receive CCM services, which can help you connect the dots so you can spend more time doing what you love. In family medicine, conditions that last a year or longer and require ongoing medical attention or limit your daily activities are typically considered ‘chronic,’ and these include:

Medicare chronic care management services provide at least 20 minutes per month of assistance for medication reviews or coordination between your pharmacy, specialists, testing centers, hospitals, or other providers, plus 24/7 access to care.
