Wilmington Health’s Collaborative Care Clinic strives to provide premium access to their Primary Care Providers through a new service offering, myPCP (my Premium Care Provider). The primary objective is to empower our patients with the ability to access their provider at the time they need them, expanding opportunities to easily communicate with their provider, as well as receive concierge-level care. myPCP is a subscription service for those patients who need access to their provider outside of standard business hours, as well as the ability to connect with their provider via a variety of digital outlets.
Ultimately the goal is to provide patients with direct and immediate access to their primary care providers to prevent them from having the need to visit urgent or high-cost care options.
Our team strives to support healthcare solutions and treatment plans that are preventive and extend wellness and improve quality of life for our patients.

- Direct 7am-9pm, 7 days a week, texting communication with your Collaborative Care provider team (MDs & ACPs)
- Direct-to-Provider phone number provided for each patient
- Guaranteed same-day scheduled, virtual, and walk-in availability within operating clinic hours
- Welcome packets provided to all new patients
- Responses to all communications within same day

The subscription is set via auto-draft payment plans of $50/month, and a 1-year contract will be established at that time. Patients are able to cancel after a 1-year term, if they desire. This subscription is not a replacement for insurance. All visits will still be billed through insurance. The subscription simply optimizes your access to your provider through direct communication and same day availability.
