The Patient Portal has medical information populated as of your last visit with Wilmington Health. To access your medical information available in the Patient Portal, select the My Chart tab at the toolbar in the top right corner of the portal. My Chart includes demographics, vitals, allergies, disease management, finalized labs, and more.
If your information is not being displayed, select the My Chart icon on the toolbar inside the portal and select Request Personal Health Record. This will send a copy of your personal health record to your portal account and will also populate your portal My Chart with your most current visit data.
Please note that results will not populate to the portal until they have been reviewed by your provider. We have attached a couple of screen shots regarding the My Chart option below.
