Step 1
Log into the patient portal.
Step 2
Select the My Chart icon on the toolbar inside the portal on the right hand side of the page and select Request Personal Health Record.
This will send a copy of your Personal Health Record, also known as a Continuity of Care document, to your Patient Portal account. It will also populate your portal My Chart with your most current visit data.
***Please note results will not populate to the portal until they have been reviewed by your provider.
Step 3
Click the Submit button on Select Request Personal Health Record.
By selecting the Submit button, the system will generate your Personal Health Record, which is also known as a Continuity of Care Document, detailing your medical history based on the information in your Wilmington Health record. An email will be sent you that an important document is waiting in your Inbox on the Patient Portal. By generating this record, it will also populate you’re My Chart, if your information is not being displayed.
