We are proud to partner with the City of Wilmington to provide your Health Risk Assessments (HRA).
HRA results will be shared via the patient portal. If you are not currently a Wilmington Health patient or have not enrolled in the patient portal, click here.
There are some different options for completing your HRA.
You can obtain a complete physical exam between March 2 and June 12, 2026, in lieu of the Health Risk Assessment. The physical exam is a more in-depth visit with the provider, including a physical examination, full review of your health and wellness, medications, and lab results.
If you would like to schedule a physical at the clinic you can call or text 910-395-3988.
If you are outside the area or already have an established primary care provider and want to submit results from your visit with your provider, please complete these forms.
If you have had an exam and labs with a Wilmington Health provider but you are missing a lab or two, just schedule your HRA with the City employee clinic and let the staff know what you are missing. They will retrieve the rest of your information from your medical record and complete any missing items rather than conducting the full HRA.
If you want a Health Risk Assessment only and not a physical exam, you can schedule a HRA at the clinic beginning March 2, 2026.
