Health Risk Assessment

We are proud to partner with the City of Wilmington to provide your Health Risk Assessments (HRA). 

Health Risk Assessment results will be shared via the patient portal. If you are not currently a Wilmington Health patient or have not enrolled in the patient portal, click here

This year, there are some different options. 

You can obtain a complete physical exam between March 1 and June 15, 2024, in lieu of the Health Risk Assessment. The physical exam is a more in-depth visit with the provider, including a physical examination, full review of your health and wellness, medications, and lab results. 

If you would like to schedule a physical at the clinic you can call or text 910-395-3988. Or, if you have an active portal you schedule by logging into your portal, select schedule appointment, for location choose “City of Wilmington–Employees Only” and for appointment type choose “complete physical exam.” Verify correct insurance information and search for available appointment dates and times that work for you. Select the day and time and submit request. You will receive a portal message once the appointment is confirmed.   

If you are outside the area or already have an established primary care provider and want to submit results from your visit with your provider, please complete these forms.

If you have had an exam and labs with a Wilmington Health provider since March 1, but are missing a lab or two, just schedule your HRA with the City employee clinic and let the staff know what you are missing. They will retrieve the rest of your information from your medical record and complete any missing items rather than conducting the full HRA. 

If you want a Health Risk Assessment only and not a physical exam, you can schedule a HRA at the clinic beginning March 1.

Or, if you have an active portal you schedule by logging into your portal, select schedule appointment, for location choose “City of Wilmington–Employees Only” and for appointment type choose “Established patient visit” and in details for reason of visit put “HRA.” Verify correct insurance information and search for available appointment dates and times that work for you. Select the day and time and submit request. You will receive a portal message once the appointment is confirmed.
