Preparing for Bariatric Surgery

PRIMARY CARE PHYSICIAN

Discuss with your primary care physician if bariatric surgery is right for you. If so, they can make a referral to our office so that a consultation can be scheduled. Some insurance programs require a medically supervised weight loss program. If this is needed, we will make those arrangements for you with our bariatric specific nutrition team. 

REQUIRED STEPS
Getting bariatric surgery approved by your insurance takes multiple steps.

If you are considering bariatric surgery or Comprehensive Weight Management Program, consult your primary care provider first to support on placing a referral. You are also welcome to self-refer and the team will advise on what or if any documentation for authorization by your insurance is needed.

The first is to complete a Medical History Questionnaire and Insurance Verification Form. Once submitted, the surgeon will review the information and the insurance coordinator will also complete a verification.

Please submit via the following options:

  • Patient portal as an attachment
  • Fax to 910-251-1420
  • Drop off at 1202 Medical Center Drive 2nd floor front desk
  • Mail to 1202 Medical Center Drive Wilmington, NC 28401 Attention: Bariatrics.
