Preparing for Bariatric Surgery

Discuss with your primary care physician if bariatric surgery is right for you. If so, they can make a referral to our office so that a consultation can be scheduled. Some insurance programs require a medically supervised weight loss program. If this is needed, we will make those arrangements for you with our bariatric specific nutrition team.

Getting bariatric surgery approved by your insurance takes multiple steps.

The first is to complete a Medical History Questionnaire and Insurance Verification Form. Once submitted, the surgeon will review the information and the insurance coordinator will also complete a verification.

Please submit via the following options:

  • Patient portal as an attachment
  • Fax to 910-251-1420
  • Drop off at 1202 Medical Center Drive 2nd floor front desk
  • Mail to 1202 Medical Center Drive Wilmington, NC 28401 Attention: Bariatrics.

Our patient representatives will contact you to schedule a consultation.

Thank you for taking the time to complete this and assisting us in facilitating the process. We look forward to hearing from you.

Please see page 4 of Bariatric Patient Handbook to learn about your next steps once you are in the bariatric program.
