Overview

"Our team believes that everyone should have a personal medical home that serves as the focal point through which all individuals regardless of age, sex, or race—receive acute, chronic, and preventive medical services. Through an ongoing relationship with a family physician in their medical home, patients can be assured of care that is not only accessible but also accountable, comprehensive, integrated, patient-centered, safe, scientifically valid, and satisfying to both patients and their physicians."

As primary care providers, our department promotes wellness care for the entire family. Our team consists of Board Certified Family Medicine Physicians, Board Certified Internal Medicine/Pediatric Physicians, and Board Certified Advance Practitioners.

If in-patient care is needed, they work collaboratively with our Hospitalist (add link) team, ensuring your care is the best possible.

In addition, the Pediatrics department, that provides care for pediatric and adolescent patients, and the Internal Medicine department, which cares for adult patients, also offers convenient care at multiple locations throughout the area.

Having a medical home is important if you visit your doctor once a year or regularly. Your medical home is your home base that collects your medical information and shares it with collaborating providers. We coordinate your care to make sure you are being treated appropriately.
