The following information is necessary to facilitate the delivery of your healthcare. Please read over this carefully.
Your health insurance is designed to help you meet the cost of your medical care. Please inform the appointment secretary of your insurance coverage when you call to schedule an appointment.
Since insurance benefits are determined by your insurance plan, it is your responsibility to understand your insurance benefits.
Please be aware that many insurance plans require that you receive medical care from “in-network” providers. If you choose to receive medical care outside of network, your benefits will likely be reduced.
If your health insurance requires a co-payment, co-insurance, and/or deductible, you are responsible for payment at the time of your scheduled service. You are also responsbile for obtaining a referral or authorization, if required by your insurance plan. If you do not have health insurance, you are responsible for full payment at the time of service.
If you have questions about your account, our Business Office staff is available to assist you Monday – Friday from 8:00 a.m. – 5:00 p.m. Please call 395-4188 and speak with one of our customer service representatives. For your convenience, we accept cash, checks, VISA, MasterCard, and Discover.

If your insurance company is not listed, please contact them to determine if your benefits will be reduced for medical care received outside of network. Your insurance company determines the financial responsibility of your medical care.
